นโยบายคุ้มครองข้อมูลส่วนบุคคล
1. นิยาม คำนิยามในนโยบายคุ้มครองข้อมูลส่วนบุคคลฉบับนี้ให้มีความหมายดังนี้
- “ท่าน” หมายถึง ผู้ใช้บริการเว็บไซต์ และ/หรือแอปพลิเคชันของเรา
- “เรา” หมายถึง บริษัท เฮ็ลท์ คอมพาเนี่ยน จำกัด ทะเบียนนิติบุคคลเลขที่ 0105564046250
- “เว็บไซต์และ/หรือแอปพลิเคชัน” หมายถึง เว็บไซต์ และ/หรือแอปพลิเคชัน ซึ่งให้บริการโดยบริษัท เฮ็ลท์ คอมพาเนี่ยน จำกัด ทั้งนี้ ในกรณีที่เป็นแอปพลิเคชัน ให้นโยบายคุ้มครองข้อมูลส่วนบุคคลนี้ใช้บังคับรวมถึงแอปพลิเคชันในส่วนที่ได้มีการเปลี่ยนแปลง ปรับปรุง อัพเดต หรือเพิ่มเติมโดยบริษัท เฮ็ลท์ คอมพาเนี่ยน จำกัด เว้นแต่แอปพลิเคชันที่ได้มีการเปลี่ยนแปลง ปรับปรุง อัพเดต หรือเพิ่มเติมดังกล่าวจะถูกบังคับใช้ตามเงื่อนไขและข้อตกลงต่างหากจากนโยบายคุ้มครองข้อมูลส่วนบุคคลนี้
- “ข้อมูลส่วนบุคคล” หมายถึง ข้อมูลเกี่ยวกับบุคคลซึ่งทำให้สามารถระบุตัวบุคคลนั้นได้ไม่ว่าทางตรงหรือทางอ้อม ตามกฎหมายว่าด้วยการคุ้มครองข้อมูลส่วนบุคคล
- “ข้อมูลส่วนบุคคลที่ละเอียดอ่อน” หมายถึง ข้อมูลส่วนบุคคลที่เกี่ยวกับเชื้อชาติ เผ่าพันธุ์ ความคิดเห็นทางการเมือง ความเชื่อในลัทธิ ศาสนาหรือปรัชญา พฤติกรรมทางเพศ ประวัติอาชญากรรม ข้อมูลสุขภาพ ความพิการ ข้อมูลสหภาพแรงงาน ข้อมูลพันธุกรรม ข้อมูลชีวภาพ หรือข้อมูลอื่นใดตามที่คณะกรรมการคุ้มครองข้อมูลส่วนบุคคลจะประกาศกำหนด
2. รายละเอียดทั่วไป
นโยบายคุ้มครองข้อมูลส่วนบุคคลนี้มีขึ้นเพื่อเป็นการชี้แจงรายละเอียดและวิธีการจัดการเกี่ยวกับข้อมูลส่วนบุคคลของผู้ใช้งาน โดยเราอาจดำเนินการปรับปรุงหรือแก้ไขนโยบายคุ้มครองข้อมูลส่วนบุคคลที่กำหนดดังต่อไปนี้ รวมทั้งที่ได้กำหนดไว้โดยเฉพาะเจาะจงอยู่ในส่วนใดส่วนหนึ่งของเว็บไซต์
และ/หรือ แอปพลิเคชันนี้ หากมี ไม่ว่าบางส่วนหรือทั้งหมดเป็นครั้งคราว เพื่อให้สอดคล้องกับแนวทางการให้บริการและหลักเกณฑ์ของกฎหมายที่มีการเปลี่ยนแปลงไป ดังนั้น ท่านจึงควรติดตามนโยบายคุ้มครองข้อมูลส่วนบุคคลที่กำหนดไว้นี้อยู่เสมอ อย่างไรก็ดี
เราจะเผยแพร่การเปลี่ยนแปลงนโยบายคุ้มครองข้อมูลส่วนบุคคลในหน้าเว็บไซต์ หรือ แอปพลิเคชันนี้ และในกรณีที่มีการเปลี่ยนแปลงในสาระสำคัญ เราจะแจ้งให้ท่านทราบ และในกรณีที่จำเป็นตามกฎหมาย เราอาจดำเนินการขอความยินยอมเพิ่มเติมจากท่าน
อนึ่ง นโยบายคุ้มครองข้อมูลส่วนบุคคลนี้มีขึ้นเพื่อบังคับใช้กับข้อมูลส่วนบุคคลที่เกี่ยวข้องกับการลงทะเบียนสมัครใช้บริการ การสมัครใช้บริการสมาชิก การใช้สินค้าและบริการ การเข้าถึงและใช้เนื้อหา ฟีเจอร์ เทคโนโลยี หรือฟังก์ชันที่ปรากฏในเว็บไซต์นี้ ตลอดจนแอปพลิเคชันของเรา
ทั้งที่มีอยู่ในปัจจุบันและที่เราจะได้พัฒนาหรือจัดให้มีขึ้นในอนาคต
3. ข้อมูลส่วนบุคคลที่เก็บรวบรวม ใช้ เปิดเผย และวิธีการเก็บรวบรวมข้อมูล
เราเก็บรวบรวมข้อมูลด้วยเทคโนโลยีต่าง ๆ เช่น คุกกี้ ซึ่งเป็นชิ้นส่วนข้อมูลเล็ก ๆ ที่เก็บอยู่ในอุปกรณ์ของท่านที่จะทำให้เว็บไซต์สามารถจดจำข้อมูลการเข้าถึงเว็บไซต์หรือวิธีที่ท่านใช้งานเว็บไซต์ในแต่ละครั้ง (ข้อมูลเพิ่มเติมเกี่ยวกับคุกกี้) โดยข้อมูลที่เกี่ยวข้องกับท่านที่เราเก็บรวบรวม
อาจจะเป็นการเก็บรวบรวมข้อมูลส่วนบุคคลโดยตรงจากท่านหรือโดยอ้อมจากแหล่งข้อมูลอื่นๆ ประกอบไปด้วย:
ข้อมูลที่ท่านให้ไว้โดยตรง: เราจะรวบรวมข้อมูลที่ท่านส่งให้กับเรา เช่น ข้อมูลที่ท่านกรอกขณะลงทะเบียนสมัครใช้บริการ ข้อมูลที่ใช้ในการสมัครใช้บริการ DR.in และข้อมูลการร่วมกิจกรรมต่าง ๆ ในเว็บไซต์หรือ แอปพลิเคชัน ข้อมูลการทำแบบสำรวจ ข้อมูลบัญชีผู้ใช้งาน (Account)
หรือข้อมูลที่ท่านได้แก้ไขปรับปรุงในข้อมูลบัญชีผู้ใช้งาน (Account) ของท่าน หรือข้อมูลที่ได้จากการที่ท่านติดต่อกับเราหรือทีมงานของเรา รวมทั้งเสียงบันทึกติดต่อสื่อสารกับเราหรือทีมงานของเรา หรือบันทึกรายละเอียดการติดต่อ หรือข้อมูลที่ได้จากบัญชีผู้ใช้งาน
(Account) อื่น ๆ ที่เรามีเหตุอันควรเชื่อได้ว่าท่านควบคุมดูแลอยู่ อันรวมถึงแต่ไม่จำกัดเฉพาะ ข้อมูลทุกชนิดที่แสดงบนหน้าประวัติผู้ใช้งานและประวัติการใช้บริการต่างๆ อาทิ ชื่อ-นามสกุล เบอร์โทรศัพท์ หมายเลขบัตรประจำตัวประชาชน ที่อยู่ เพศ
ความสนใจ และความเห็นทุกอย่างที่ท่านได้แสดงผ่านเว็บไซต์ หรือ แอปพลิเคชัน (ถ้ามี) เพื่อจัดเก็บไว้กับบัญชีผู้ใช้งาน (Account)
ข้อมูลที่ได้รับจากการใช้บริการของท่านและที่อาจเก็บรวบรวมจากแหล่งข้อมูลอื่นๆ: เราจะรวบรวมข้อมูลเกี่ยวกับบริการที่ท่านใช้และวิธีการใช้งานของท่าน ซึ่งข้อมูลเหล่านี้รวมถึงแต่ไม่จำกัดเฉพาะ ข้อมูลอุปกรณ์ที่ท่านใช้สำหรับการเข้าใช้งานเว็บไซต์ หรือ แอปพลิเคชัน
ข้อมูลการนัดหมาย สิทธิการรักษา ประวัติการชำระเงิน และ/หรือ ข้อมูลจราจรทางคอมพิวเตอร์ (Log) ข้อมูลการติดต่อและสื่อสารระหว่างท่านและผู้ใช้งานรายอื่น และข้อมูลจากการบันทึกการใช้งาน เช่น ตัวระบุอุปกรณ์ หมายเลข IP ของคอมพิวเตอร์ รหัสประจำตัวอุปกรณ์
ประเภทอุปกรณ์ ข้อมูลเครือข่ายมือถือ ข้อมูลการเชื่อมต่อ ข้อมูลตำแหน่งที่ตั้งทางภูมิศาสตร์ ประเภทของเบราว์เซอร์ (Browser) ข้อมูลบันทึกการเข้าออกเว็บไซต์ ข้อมูลเว็บไซต์ ที่ผู้ใช้งานเข้าถึงก่อนและหลัง (Referring Website) ข้อมูลบันทึกประวัติการใช้เว็บไซต์
ข้อมูลบันทึกการเข้าสู่ระบบ (Login Log) ข้อมูลรายการการทำธุรกรรม (Transaction Log) พฤติกรรมการใช้งาน (Customer Behavior) สถิติการเข้าเว็บไซต์ หรือแอปพลิเคชัน เวลาที่เยี่ยมชมเว็บไซต์หรือแอปพลิเคชัน (Access Time) ข้อมูลที่ท่านค้นหา การใช้ฟังก์ชันต่าง
ๆ ในเว็บไซต์ หรือแอปพลิเคชัน และข้อมูลที่เราได้เก็บรวบรวมผ่านคุกกี้หรือเทคโนโลยีอื่นที่คล้ายกัน
ข้อมูลส่วนบุคคลที่ละเอียดอ่อน: ข้อมูลสุขภาพ (เช่น ข้อมูลการแพ้ยา ข้อมูลการรักษา ข้อมูลประวัติการใช้ยา และประวัติการเจาะเลือด)
ทั้งนี้ รายละเอียดตามข้างต้นเป็นตัวอย่างข้อมูลที่เราอาจจะเก็บรวบรวม โดยเราจะเก็บรวบรวมข้อมูลส่วนบุคคลของท่านเฉพาะข้อมูลที่จำเป็น และในระยะเวลานานเท่าที่จำเป็นเพื่อประโยชน์ในการใช้งานข้อมูลส่วนบุคคลตามที่ระบุไว้ในข้อ 4 เท่านั้นหรือตามที่กฎหมายกำหนดหรืออนุญาตให้เราเก็บรักษาข้อมูลส่วนบุคคลไว้นานกว่านั้นได้
หากท่านปฏิเสธที่จะให้ข้อมูลส่วนบุคคลที่จำเป็นในการจัดหาผลิตภัณฑ์ หรือบริการตามที่ท่านร้องขอ เราอาจไม่สามารถเสนอหรือจัดหาผลิตภัณฑ์และให้บริการของเราให้แก่ท่านได้ ซึ่งอาจกระทบถึงการเข้าทำสัญญาหรือปฏิบัติหน้าที่ตามสัญญาที่มีกับท่าน และอาจส่งผลกระทบต่อท่านทำให้ท่านไม่ได้รับความสะดวกในการเข้าถึงผลิตภัณฑ์
หรือใช้บริการของเราทั้งหมดหรือแต่บางส่วน รวมทั้งอาจส่งผลกระทบต่อการปฏิบัติตามกฎหมายใดๆ ที่เรามีหน้าที่ต้องปฏิบัติตาม
4. การใช้งานข้อมูลส่วนบุคคล
4.1 วัตถุประสงค์ในการเก็บรวบรวม ใช้ และเปิดเผยข้อมูลส่วนบุคคล เราเก็บรวบรวม ใช้ และเปิดเผยนำข้อมูลส่วนบุคคลของท่านเพื่อวัตถุประสงค์ดังต่อไปนี้
- เพื่อให้การใช้บริการเป็นไปด้วยความเรียบร้อยและสอดคล้องกับกฎหมาย หลักเกณฑ์ และระเบียบต่าง ๆ ที่เกี่ยวข้อง รวมถึงเพื่อการปฏิบัติหน้าที่ตามกฎหมาย และกฎเกณฑ์ที่เกี่ยวข้องหรือใช้บังคับกับเรา ทั้งที่มีผลใช้บังคับในปัจจุบันและที่อาจจะมีการแก้ไขหรือเพิ่มเติมในอนาคต
- เพื่อประโยชน์ในการยืนยันหรือระบุตัวตนของท่านเมื่อเข้าใช้งานบริการต่างๆ อาทิ บริการสมาชิก
- เพื่อจัดหาผลิตภัณฑ์และให้บริการแก่ท่านบนเว็บไซต์ และ/หรือแอปพลิเคชันของเราตามที่ท่านร้องขอ
- เพื่อตรวจสอบข้อมูลการใช้บริการของท่านเพื่อการพัฒนามาตรฐานความมั่นคงปลอดภัยในการใช้บริการ การจัดการและการคุ้มครองโครงสร้างพื้นฐานทางเทคโนโลยีสารสนเทศ โดยในส่วนนี้เราอาจนำข้อมูลส่วนบุคคลของท่าน เพียงเท่าที่จำเป็นและอาจดำเนินการให้มีการเข้ารหัส (Encrypt) ก่อนนำไปใช้และ/หรือจัดให้มีการสุ่มตรวจ การทดสอบการเข้าใช้งานโดยบุคคลอื่นเพื่อนำไปใช้ในการบริหารจัดการความเสี่ยง ตรวจจับ ป้องกัน หรือขจัดการฉ้อโกง หรือกิจกรรมอื่น ๆ ที่มีแนวโน้มว่าจะเป็นการละเมิดกฎหมาย ระเบียบการใช้งานที่เกี่ยวข้อง หรือข้อตกลงและเงื่อนไขการใช้งานเว็บไซต์ (“ข้อตกลงและเงื่อนไขการใช้”) ของเรา
- เพื่อเพิ่มประสิทธิภาพในการให้บริการงานด้านต่าง ๆ แก่ท่านมากยิ่งขึ้น
- เพื่อติดต่อท่าน ผ่านทางโทรศัพท์ ข้อความ (SMS) อีเมล หรือไปรษณีย์ หรือผ่านช่องทางใด ๆ เพื่อสอบถาม หรือแจ้งให้ท่านทราบ หรือตรวจสอบและยืนยันข้อมูลเกี่ยวกับบัญชีของท่าน หรือสำรวจความคิดเห็น หรือแจ้งข้อมูลข่าวสารอื่นใดที่เกี่ยวข้องกับการให้บริการของเราตามที่จำเป็น
- เพื่อประโยชน์อื่นใดที่เกี่ยวข้องกับการดำเนินธุรกิจของเรา เช่น เพื่อประโยชน์ในการศึกษา วิจัย จัดทำสถิติ และพัฒนาการให้บริการ
- เพื่อจัดทำการตลาดหรือการโฆษณาเป้าหมายที่เกี่ยวข้อง รวมถึงการจัดส่งเนื้อหา การโฆษณาประชาสัมพันธ์ กิจกรรมและโปรโมชันต่างๆ ตลอดจนการให้คำแนะนำต่าง ๆ ที่เหมาะสมเพื่อให้การให้บริการต่าง ๆ ตรงกับความสนใจของท่าน
4.2 ฐานทางกฎหมายในการเก็บรวบรวม ใช้ เปิดเผย และ/หรือโอนข้อมูลส่วนบุคคลเราอาศัยฐานทางกฎหมายดังต่อไปนี้ในการเก็บรวบรวม ใช้ และเปิดเผยข้อมูลส่วนบุคคลของท่าน
- กรณีมีความจำเป็นสำหรับการปฏิบัติตามสัญญาที่ท่านเป็นคู่สัญญากับเรา หรือเพื่อดำเนินการขั้นตอนต่างๆ ตามที่ท่านร้องขอก่อนจะเข้าทำสัญญากับเรา
- กรณีมีความจำเป็นเพื่อประโยชน์โดยชอบด้วยกฎหมายของเราหรือบุคคลภายนอก โดยที่ประโยชน์ดังกล่าวไม่เกินกว่าสิทธิขั้นพื้นฐานในข้อมูลส่วนบุคคลของท่าน
- กรณีมีความจำเป็นเพื่อการปฏิบัติหน้าที่ในการดำเนินภารกิจเพื่อประโยชน์สาธารณะของเรา หรือปฏิบัติหน้าที่ในการใช้อำนาจรัฐที่ได้มอบให้แก่เรา
- กรณีมีความจำเป็นที่เราต้องปฏิบัติตามกฎหมาย
- กรณีที่มีความจำเป็นในการดำเนินการป้องกันหรือระงับอันตรายต่อชีวิต ร่างกาย หรือสุขภาพของบุคคล
- ความยินยอมหรือความยินยอมโดยชัดแจ้งจากท่านในกรณีที่มีความจำเป็นที่เราต้องขอความยินยอม
5. ข้อมูลส่วนบุคคลเกี่ยวข้องกับ Covid-19 ที่ถูกเก็บรวบรวม
แอพลิเคชัน DR.in สร้างมาเพื่ออำนวยความสะดวกในการเข้าถึงบริการตรวจ COVID-19 แบบ RT-PCR ได้ในราคาพิเศษ และขอรับคำปรึกษาจากเภสัชกรผ่านโทรทางไกล (Video Conference) รวมไปถึงดูแลผู้ป่วยรูปแบบ Home isolation และรับรองไปถึงการบันทึกผลการตรวจ Test kit เพื่อออก “My Pass” สำหรับองค์กรที่ต้องการขอตรวจ Test kit ก่อนเข้าสถานที่ชุมชน อาทิ บริษัท โรงเรียน ฯลฯ ซึ่งแอพลิเคชัน DR.in ถูกออกแบบโดยบริษัท เฮ็ลท์ คอมพาเนี่ยน จำกัด ร่วมกับ CU Pharmacy Enterprise (CUPE)
DR.in เป็นแอพของบริษัท เฮ็ลท์ คอมพาเนี่ยน จำกัด ร่วมกับ CU Pharmacy Enterprise (CUPE)ในประเทศไทย ทำหน้าที่ดูแลผู้ป่วยระยะไกล ผ่านการโทรทางไกล (Video Conference) เพื่อป้องกันและคัดกรองโรค ของกระทรวงสาธารณสุขสำหรับข้อมูลที่เกี่ยวข้องกับการตรวจ Covid-19
แอพลิเคชัน DR.in สร้างขึ้นสำหรับการช่วยเหลือผู้มีเชื้อ COVID-19 สามารถเข้าถึงการรักษาได้อย่างทันท่วงทีและการใช้แอพนี้เป็นไปโดยสมัครใจ
เพื่อช่วยให้ผู้ป่วยผู้มีเชื้อไวรัส COVID-19 เข้าถึงการรักษาแบบทางไกลอย่าง Home isolation ได้ รวมไปถึงการติดตามอาการโดยเภสัชกรผู้ดูแล ภายใต้การกำหนดมาตราฐานและขั้นตอนการให้บริการเภสัชกรรมทางไกล พ.ศ. 2563 และกรมควบคุมโรคติดต่อ พ.ศ. 2558 ซึ่งแอพจะขอเก็บข้อมูลดังต่อไปนี้:
- ขอเก็บข้อมูล : เลขบัตรประจำตัวประชาชน, ชื่อ, นามสกุล, เพศ, คำนำหน้า และหมายเลขโทรศัพท์ รวมถึงรูปถ่ายบัตรประชาชน และรูปถ่ายคู่บัตร มีความสำคัญอย่างยิ่งสำหรับเราในการยืนยันตัวบุคคลและแสดงบันทึกผลการตรวจทางห้องปฏิบัติการแบบ RT-PCR รวมถึงบันทึกผลตรวจจากการที่ผู้ใช้งานตรวจด้วย Test Kit ซึ่งข้อมูลเหล่านี้ถูกรวบรวมเพื่อวัตถุประสงค์ในการดูแลรักษากรณีผู้ป่วยตรวจหา COVID-19 แล้วพบเชื้อเพื่อระบุตัวตนได้อย่างถูกต้อง และอนุญาตให้เภสัชกรของเราติดต่อเพื่อวัตถุประสงค์ในการติดตามผลและสั่งยารวมถึงอุปกรณ์ทางการแพทย์เพื่อรักษาดูแลอาการผ่านระบบโทรทางไกล (Video Conference) รวมถึงข้อมูลเหล่านี้มีการเข้ารหัสและป้องกันเป็นอย่างดี
- รวบรวมที่อยู่ (จังหวัด อำเภอ ตำบล) เพื่อเลือกสถานที่จัดส่ง ยา หรืออุปกรณ์ทางการแพทย์ กรณีผู้ป่วยมีความจำเป็นต้องใช้สำหรับบรรเทาอาการป่วยจาก COVID-19
- ID ที่อยู่บัตรประจำตัวประชาชน และรูปภาพบัตรประชาชนของคุณถูกป้อนเพื่อใช้ยืนยันตัวตนตนของคุณเท่านั้น และถูกลบทิ้งเมื่อสิ้นสุดกระบวนการ หรือภายใน 10 ปีระบบจะทำการลบข้อมูลส่วนตัวทิ้ง ขั้นตอนนี้มีความสำคัญอย่างยิ่งในการป้องกันการฉ้อโกงข้อมูลประจำตัว
- บัตรผ่านหรือ My Pass เป็นการแสดงสถานะการติดเชื้อที่ผู้ใช้งานต้องมีการลงประวัติการตรวจ Test Kit โดยการเข้าเมนู “เริ่มการตรวจ Rapid Test” => เลือก “ตรวจด้วยตนเอง” จากนั้นถ่ายภาพผลตรวจและเลือกผลการตรวจ และทำการบันทึกผลตรวจ
- แอปพลิเคชันมีการเก็บตำแหน่ง เพื่อหาร้านยาใกล้ตัวของคุณเพื่อที่จะอำนวยในการขอคำปรึกษาอาการป่วย และความสะดวกเมื่อผู้ป่วยต้องการยาหรืออุปกรณ์ดำรงชีพให้ส่งถึงมืออย่างรวดเร็วที่สุด
- แอปพลิเคชันมีการเข้าถึง Audio, image และ Record audio เพื่อให้บริการโทรทางไกล (Video Conference) ระหว่างผู้ใช้งานและเภสัชกรผู้ให้คำปรึกษา ในกรณีผู้ใช้งานต้องการขอคำปรึกษาอาการไม่พึงประสงค์ หรือต้องการสอบถามข้อมูลเพิ่มเติมเกี่ยวกับโรค Covid-19 โดยระบบจะมีการบันทึกเสียง ภาพ และวีดีโอ เพื่อเป็นหลักฐานทางอิเล็กทรอนิกส์ เพื่อปกป้องผู้ใช้งาน จากเหตุการณ์ไม่พึงประสงค์
การแบ่งปันข้อมูล
- เก็บรวบรวมร่วมกับบุคลากรทางการแพทย์เพื่อวัตถุประสงค์ด้านสาธารณสุข ข้อมูลจะไม่ถูกแชร์เพื่อวัตถุประสงค์ทางการค้าโดยไม่ได้รับอนุญาตจากคุณ
- ข้อมูลที่รวบรวมได้เพื่อช่วยเราดูแลผู้ป่วยทางไกล (Home Isolution) ที่อยู่ที่บ้านผ่านระบบโทรทางไกล (Video Conference) เพื่อป้องกันไม่ให้ผู้ป่วยมีอาการรุนแรงขึ้น เพื่อลดจำนวนผู้ป่วยสีเขียวที่จะเปลี่ยนผู้ป่วยสีเหลืองหรือสีแดง และจำเป็นต้องเข้ารักษาตัวในโรงพยาบาล

6. การขอสิทธิ์การเข้าถึงของแอปพลิเคชันในโทรศัพท์
แอปพลิเคชัน DR.in จะขอสิทธิ์การเข้าถึงฟีเจอร์ต่างๆ ในโทรศัพท์ เช่น กล้องถ่ายรูปหรือไมโครโฟน แอปจะส่งการแจ้งเตือนเพื่อขอสิทธิ์ใช้ฟีเจอร์ต่างๆ ในโทรศัพท์ ซึ่งคุณจะอนุญาตหรือปฏิเสธก็ได้ โปรดจงทราบว่าหากปฏิเสธอาจจะส่งผลกระทบต่อการใช้งานแอปพลิเคชันของคุณในบางส่วน นอกจากนี้คุณยังสามารถเปลี่ยนแปลงการให้สิทธิ์แอปหรือตามประเภทของสิทธิ์ในการตั้งค่าของโทรศัพท์ได้ด้วย ด้านล่างนี้เป็นรายการสิทธิ์และการดำเนินการของแต่ละสิทธิ์เมื่อเปิดไว้ในแอปพลิเคชัน:
- กล้องถ่ายรูปและวีดีโอ : เข้าถึงกล้องและวีดีโอเพื่อใช้บริการปรึกษาเภสัชกรผ่านทางดิโอคอล ในการเข้ารับการปรึกษากับเภสัชกรในการสั่งซื้อ Test Kit และการขอคำแนะนำจากเภสัชกรในกรณีต่างๆ ตามสถานการณ์ส่วนบุคคล
- ไมโครโฟน : แอปพลิเคชันขอสิทธิ์การเข้าถึงไมโครโฟนเพื่อใช้ในการให้เสียงการสนทนา/โต้ตอบ ขณะเข้ารับการปรึกษากับเภสัชกร
- ไฟล์และสื่อ : แอปพลิเคชันขอสิทธิ์การเข้าถึงไฟล์และสื่อเพื่ออัปโหลดรูปภาพของผลการทดสอบ Test Kit เพื่อให้เภสัชกรแนะนำวิธีรักษาอาการด้วยตนเอง การทำ Home Isolation และคำแนะนำให้คุณทดสอบด้วยวิธี RT-PCR ต่อไป
- บันทึกวิดีโอ : การบันทึกวิดิโอการสนทนาระหว่างคุณและเภสัชกร โดยระบบจะมีการบันทึกเสียง ภาพ และวีดีโอ เพื่อเป็นหลักฐานทางอิเล็กทรอนิกส์ เพื่อปกป้องผู้ใช้งาน จากเหตุการณ์ไม่พึงประสงค์
- SMS : ในการเข้าใช้งานครั้งแรกจะเราส่งข้อความ SMS (ข้อความ) เพื่อระบุ OTP ในการเข้าใช้งานของท่าน
- ตำแหน่ง : แอปพลิเคชั่นมีการเข้าถึงตำแหน่งของท่าน ผ่านการใช้งาน WiFi หรือข้อมูลมือถือ (หรือทั้งสองอย่าง) เพื่อระบุตำแหน่งของอุปกรณ์ ใช้หาร้านยาใกล้ตัวของท่านในการอำนวยการขอคำปรึกษาอาการป่วย และใช้สำหรับเพิ่มสมุดที่อยู่ของผู้ใช้งาน เพื่อความสะดวกผู้ป่วยกรณีต้องการยาหรืออุปกรณ์ดำรงชีพให้ส่งถึงมืออย่างรวดเร็วที่สุด
- Bluetooth : แอปพลิเคชั่นมีการจับคู่ Bluetooth กับอุปกรณ์เสริม Smart Watch เพื่อการเข้าถึงข้อมูลของนาฬิกา โดยนำมาวิเคราะห์ และแสดงผลในแอปพลิเคชั่นสำหรับผู้ใช้งาน
7. การเปิดเผยข้อมูลส่วนบุคคลต่อบุคคลภายนอก
เพื่อประโยชน์ในการให้บริการตามเว็บไซต์หรือแอปพลิเคชันนี้ เราอาจเปิดเผยข้อมูลส่วนบุคคลของท่านให้กับบริษัทในกลุ่มธุรกิจเรา หรือพันธมิตรซึ่งทำงานร่วมกับเรา หรือผู้ให้บริการอื่นทั้งในและต่างประเทศ เช่น บุคคลที่เราได้ว่าจ้างให้ดำเนินงานที่เกี่ยวข้องกับข้อมูลส่วนบุคคล

9. การเข้าถึงและการปรับปรุงข้อมูลส่วนบุคคล
- ท่านสามารถเปลี่ยนแปลงข้อมูลส่วนบุคคลของท่าน ในส่วนของข้อมูลที่ท่านได้ให้ไว้ ผ่านการสมัครใช้บริการสมาชิกได้ โดยการเข้าแอปพลิเคชันหรือเว็บไซต์ แล้วเข้าสู่ระบบ (Login) และไปยังเมนู “ตั้งค่า” > “ข้อมูลส่วนตัว” เพื่อแก้ไขข้อมูลให้ถูกต้อง รวมถึงสามารถตั้งค่าการใช้งานในส่วนต่างๆ ได้ด้วยตนเอง
- ท่านสามารถยกเลิกการรับข่าวสาร ได้ทางแอปพลิเคชันหรือเว็บไซต์ ที่ท่านได้รับจากเรา โดยการเข้าสู่ระบบ (Login) และไปยังเมนู “ตั้งค่า” > “เพิกถอนความยินยอม”
- ท่านสามารถดำเนินการลบข้อมูลส่วนบุคคลของท่านออกจากระบบ โดยการเข้าสู่ระบบ (Login) และไปยังเมนู “ตั้งค่า” > “ยกเลิกบัญชี” เพื่อลบข้อมูลทั้งหมดของท่าน ข้อมูลทั้งหมดของท่านจะถูกลบอย่างถาวร ในระหว่างนั้น ท่านจะไม่สามารถใช้งานบัญชีของท่านอีกครั้งได้ และ แอปพลิเคชันที่ใช้บัญชีของท่าน จะไม่สามารถเข้าถึงได้อีกต่อไป รวมถึงทางเราไม่สามารถดึงข้อมูลต่างๆที่ได้ทำการลบไปแล้วกลับมาได้
ทั้งนี้ ในกรณีที่ท่านไม่ยินยอมให้เราใช้ข้อมูลส่วนบุคคลของท่าน สำหรับกรณีที่ต้องได้รับความยินยอม หรือท่านได้ดำเนินการลบข้อมูลส่วนบุคคลของท่านออกจากระบบของเรา อาจทำให้ท่านไม่สามารถรับบริการจากเราได้ หรืออาจทำให้บริการที่ท่านได้รับจากเราไม่มีประสิทธิภาพเท่าที่ควร
เราจะพยายามอย่างเต็มที่ตามความสามารถของระบบงานที่เกี่ยวข้องเพื่ออำนวยความสะดวกและดำเนินการตามคำร้องขอของท่าน เว้นแต่จะปรากฏข้อเท็จจริงว่า การดำเนินการตามคำร้องขอนั้นเสี่ยงต่อการละเมิดนโยบายความเป็นส่วนตัวของผู้ใช้งานรายอื่น หรือเป็นการขัดต่อกฎหมาย หรือนโยบายความปลอดภัยของระบบ หรือกรณีที่เป็นการพ้นวิสัยในทางปฏิบัติตามคำร้องขอของผู้ลงทะเบียน
